Obstetric ultrasonography

At seven weeks pregnant, your baby is about the size of a blueberry! Since most women don’t know the exact date of conception, the standard method for charting a pregnancy is by counting forward from the first day of your last menstrual period LMP. This method assumes that you ovulate 14 days after your period begins and that you’ll remember the day your period started! These results give health care providers—and you—a ballpark estimate for your baby’s due date. For more accurate pregnancy dating, health care providers can request an ultrasound. Careful measurements taken in the first trimester are even more accurate at predicting an embryo’s age than ultrasounds performed later in pregnancy. Using these measurements, a sonographer can pinpoint a baby’s age to within a few days. As the baby-to-be grows larger, these measurements are less reliable at predicting a baby’s age because of varying growth rates.

First trimester worries

Whilst you may get a positive pregnancy test before your period is even due it is a couple of weeks before the pregnancy is big enough to be seen on scan. In the early weeks how much we can see in very early pregnancy is different for every woman and every pregnancy. At 4 weeks gestation 2 weeks after conception when you have just missed your period we cannot detect the pregnancy on scan, all we can see is that the lining of the womb has become thicker. Somewhere between 4 and 5 weeks, we can sometimes see a small black fluid area in the uterus but we cannot confirm it is a pregnancy sac until we can see something growing inside it.

During the 5th week we can usually see a small pregnancy sac around 6mm and within it we can usually see a yolk sac.

For transvaginal ultrasound you can have an empty bladder By 6 to 7 weeks gestation the fetus is clearly seen on trans-vaginal ultrasound and the if the early dating scan calculates the EDD to be within 5 days of the EDD from your last.

The pregnancy is calculated based on the average menstrual cycle which is 28 days. The first day of menstrual bleeding marks the first day of the menstrual cycle. Right in the middle of this picture perfect 28 day cycle is when ovulation occurs day 14 of the cycle and roughly 2 days before and after is when you are most fertile. This is why many fertility doctors recommend to have intercourse every 3 days if you really are trying to have a baby because the ovulation may occur a little earlier or later than you may think.

When calculating the weeks of pregnancy we include those two weeks from the first day of the LMP as this is the easiest part of the cycle to notice and to record. We also will enquire about the length of your cycle and whether it is regular or irregular. All of this is helpful in trying to establish the gestation. For example, if your cycle is regular and 33 days long we would expect your ovulation to have occurred later 5 days and an early pregnancy scan will most likely show a baby that measures 5 days less than you were expecting.

Now to answer your question, it may be that you ovulated later in the cycle or have a longer cycle.

What will I see at a 5 week early pregnancy scan, why should I wait until 7 weeks?

First-trimester ultrasound

First trimester scanning is useful to identify abnormalities in the early development of a pregnancy, including miscarriage and ectopic pregnancy, and provides the most accurate dating of a pregnancy. Technique First trimester scanning can be performed using either an abdominal approach or a vaginal approach. Abdominal scanning is performed with a full maternal bladder, provides a wider field of view, and provides the greatest depth of view.

Vaginal scanning is best performed with the bladder empty, gives a much greater resolution with greater crispness of fine detail. In circumstances where both approaches are readily available, the greater detail provided by transvaginal scans usually outweighs other considerations, and is preferred.
